Polk’s health insurance package includes the following:
Medical Insurance
Medical coverage is offered to all permanent full-time employees and their eligible dependents. Employees may choose from three Preferred Provider Organization (PPO) plans that include a $10 co-pay for routine office visits. Benefits range from 100% coverage to 80% coverage for hospitalization and specialty care. Employee premiums are based on the plan option selected.
Prescription Drug Program
Employees enrolled in Polk’s medical program are automatically enrolled in our prescription drug program. The co-pay for an approved generic drug is $7 at retail and $14 (for a 3-month supply) through our mail order program. Name brand medications are $20 at retail and $40 (for a 3-month supply) through our mail order program.
Flexible Spending Accounts
Polk offers Flexible Spending Accounts to regular full-time employees. Health Care and Dependent Care Flexible Spending Accounts allow employees to take advantage of tax savings by contributing pre-tax dollars into these accounts (annual limits apply) for child care or adult care expenses and non-reimbursed health care expenses that the employee or his/her eligible dependents incur including some over-the-counter medications.
Dental
Polk’s dental plan provides up to 100% coverage for preventative services and allows employees and their eligible covered dependents the freedom to choose either a participating or a non-participating dentist.
Vision
Vision is offered free to eligible employees and their eligible dependents. Services may be covered up to 100% if provided by a participating provider.
Health Care Privacy
Polk is committed to protecting the privacy of all employees and their dependents covered under our health care programs, according to the Health Insurance Portability and Accountability Act (HIPAA). HIPAA includes requirements to ensure that individual health information is kept confidential by medical providers, employers, health plans and plan administrators.
